questionnaire (noun)

Meaning –a set of printed or written questions with a choice of answers, devised for the purpose of a survey or a statistical study.

Example of using it in a sentence: "Have you answer the questionnaire, yet?.”

Synonyms: form, test, exam, quiz, opinion poll

Spelling Tip – most of you have no problems with the spelling of the word ‘question’. You have taken on board that the root word ‘quest’ ends with a ‘t’ so it is a ‘tion’. The trouble comes with the double ‘n’ and the final ‘e’.
